REGARDING THE LIFE CARE RETIREMENT HOUSING FACILITY

2. The facilities will meet the special need of elderly - safety features - grab bars, emergency alarm system, fire safety etc.
   
   
3. Emergency aid and all health needs, including an examination room will be provided in the medical unit which will be supervised by registered nurses, assisted by licensed practical nurses and aides. The medical program will be under the direction of medical doctors, which will be assisted by a consulting staff as needed. This is all to be arranged and discussed with the local community doctors on staff at the local hospital and clinics.
   
   
4. Staff - A full time, 24-hour staff will be on duty with registered nurse and trained, licensed technicians.
   
   
5. Monthly charges will be set at level within the financial reach of a significant segment of the community's elderly persons.
   
   
6. Admission requirements to be proposed by the following plan:
   An entrance fee, and pay a monthly service fee and be cared for through life on the Life Contract.
   
   
7. Eligibility for Admittance - Age will be 55 and older. Must pass an examination by the Center's Medical Director and should be in fairly good physical and mental health. Guidelines used to determine admissions to be developed by an Admissions Committee under the direction of the Board of Directors. Inquiries and applications for admission will be received and processed by a trained Social Worker and reviewed by the Administrator.
   
   
8. Description of the facility -Total number of units is estimated at a minimum of 20. Depending on the need, there will be one bedroom, two bedroom and efficiency apartments. All with kitchen facilities, patios, raised gardens, and pets are allowed. Additional amenities are: Barber/Beauty shop, gift shop, Craft room, dining room for communal dining room and medical facilities.

REGARDING THE MEDICAL CARE CENTER

Lilac Villa is one of few retirement communities that is to have an attached skilled care licensed Health Care Center-where an experienced nursing staff will be present, not only during the day, but at every hour around the clock. The Center will also be licensed to provide home health care and staffed by professionals.

The emphasis at Lilac Villa will be placed on preventive health care to maintain wellness. The Health Care Program will provide all residents with health services such as diet instruction, exercise programs, wellness seminars, physical therapy, periodic checks of blood pressure and other programs designed to maintain your health.

1. Health Services Provided:
   • Emergency and consultative nursing
   • Catered Living residence (independent living with assistance)
   • On site assisted living care and skilled nursing care
   There will be a call system in the bedrooms, and bathrooms of every apartment, the part of the home, where most medical emergencies arise. These devices will be constantly monitored at a nursing station in the Health Care Center and can be used at any time to summon trained medical help to your apartment in an emergency.
   Other services to be provided: An eye, ear, nose and throat specialist and a dentist to visit weekly or on an as needed basis. A podiatrist will provide care for Residents who need the services. Physical therapy to be provided in a well equipped physical therapy department. The medical program will also include services in the local hospital when considered necessary by the center's physicians.
   The Medical Care Center services will be paid for in the form of your entrance fee and monthly service fee. Of course, prescriptions and personal supplies, as well as any fees charged by doctors and specialists, are the resident’s responsibility. In addition, second and third meals each day are charged to residents.
   
   
2. Doctors:
   Your relationship with your personal physician need not change at all, assuming that your physician is in the area. Lilac Villa is required by law to ensure that you designate a physician who would be ready to attend you immediately if you enter the Health Care Center. Lilac Villa requests that you make this arrangement when you become a resident so that the information will be on record. The residents deal with their physicians just as they always have, retaining their own doctors and paying for the services they provide. The Health Care Center makes available, at no cost to residents, a medical clinic where appointments can be made with a private physician.
   
   
3. Medical Insurance:
   All residents are required to have Medicare parts A & B, and one supplemental insurance policy- the amount of coverage which is considered sufficient in case hospitalization would be necessary. Residents may convalesce in the Health Care Center following hospital treatment. This is also one of the services provided for our residents.
